• Please bring your insurance card with you each appointment

INITIAL APPOINTMENTS

• Require a minimum of a 60 minute appointment which includes an interview, assessment, treatment, home care and reassessment

FOLLOW UP APPOINTMENTS

• Include assessment, treatment, home care and reassessment

PAYMENT

• Credit Card, E-transfer, or Cash (exact change) at beginning of appointment

• Direct billing available for a majority of insurance companies

• Receipt will be emailed to client within a timely manner after your appointment

CANCELLATIONS

• Appointments cancelled within 24 hours of the appointment or appointments which are no-showed will be billed for the full amount of the appointment. Your appointment time is important to us and is reserved for you. If you have an emergency, please let us know so that we can treat your specific situation with personal attention.

• If there is a third party involved, the client must submit their receipt for reimbursement

• Unfortunately, we do not accept WCB clients
